David Daniels, 49, of Columbia, formerly of Perry County, died Thursday, March 27, 2014 at the Norton Hospital in Louisville, KY. Funeral services will be Monday, March 31, 2014 at 10amCT/11amET at Maggard's Mountain View Chapel, 352 Roy Campbell Drive, Hazard, KY, with Rev. Mike Barnett officiating Visitation from 6-9pm Sunday, March 30, 2014, at the funeral home.


David Daniels, 49, Adair Co., KY (1964-2014)

He was a son of the late Orlean Wooton Daniels and is survived by his father, Sam Daniels, of Hazard, KY. He was born December 4, 2014, in Hazard, Perry County, KY. He died Thursday, March 27, 2014, in Louisville, Jefferson County, KY, at the age of 49 years, 3 months, and 23 days.

In addition to his father, he is survived by the following:
  • His wife: Sally Daniels of Columbia, KY

  • Three daughters: Ashley, Melissa and Sarah Daniels all of Columbia, KY

  • Two sisters: Judy Williams of Hazard, KY, and Brenda Hash of Campbellsville, KY

  • Two brothers: Mike Milam and James Milam both of Hazard, KY
