Collision in KSP Post 15 area, at Springfield, KY, takes life Lebanon, KY, driver pronounced dead at scene. Other driver airlifted to UL Hospital. Two passengers airlifted to Kosair Children's Hospital. Another passenger transported to Spring View Hospital in Lebanon and later transferred to Kosair Children's Hospital in Louisville, KY. KSP Post 15, Columbia, KY, Trooper Fred Cornett assisted by Post 15 Trooper Morris Farris, Washington County EMS, and Washington County Fire and Rescue By TFC Wm. Gregory, Public Affairs Officer Kentucky State Police Post 15, Columbia, KY WASHINGTON CO., KY - Trooper Fred Cornett is investigating a fatal collision that occurred Tuesday, August 24, 2011, at approximately 4:25pmET, 2 miles south of Springfield on KY 55. The collision occurred when a 1999 Toyota, operated by Bobby C Perry, age 71, of Springfield, was traveling northbound on KY55, crossed the center line and struck a 2008 Honda, operated by Setsuko Obata, age 50, of Lebanon, head on. Perry was pronounced deceased at the scene. Obata was flown from the scene of the collision to the University of Louisville Hospital for her injuries. Two passengers in the Obata vehicle, Sara Obata, age 16, and Noel Obata, age 14, were airlifted to Kosair Children's Hospital. A third passenger in the Obata vehicle, Louis Obata, age 12, was transported by Washington County EMS to Spring View Hospital and later transferred by ground to Kosairs; All subjects were belted at the time of the collision. Trooper Cornett was assisted at the scene by Trooper Morris Farris, Washington County EMS and Washington County Fire & Rescue.
