Adair Co. Health Dept. will hold school physicals clinic

By appointment only; fees will be on sliding scale

The Adair County Health Department, 801 Westlake DR, Columbia, KY will hold a special clinic on July 15, 2011 for students needing school physicals. The clinic will be by appointment only. Appointments will be available 8am-3pmCT.



Students entering school for the first time, as well as students entering sixth grade are required to have a physical exam and updated immunizations prior to the beginning of the school year. Effective July 1, 2011, new requirements for immunizations have been added for both entry and sixth grade levels. All immunizations can be given on the same day as the physical.

The charges for the exam are based on a sliding fee scale according to the number in the household and annual gross income. Exams are also covered by Medicaid/KCHIP.

All students receiving a physical at this clinic will be eligible to register for door prizes to be given away, including a bike and a MP3 player.
