2011-2012 Kindergarten registration April 15 & April 22, 2011

Parents are urged to plan ahead kindergarten registration. A few simple to complete - but absolutely essential - requirements are listed below, along with help numbers

On Friday, April 15, 2011, and Friday, April 22, 2011 the Adair County School District will hold registration from 8am to 2pmCT at Colonel William Casey Elementary School, 222 Gen. John Adair DR, Columbia, KY, for all 2011-2012 kindergarten students entering school in August. Students do not have to be present at registration.

To be eligible to attend kindergarten a students must be five (5) on or before October 1, 2011.



Parents are to bring the following documents the day of registration:
  • Student's certified birth certificate (a hospital certificate is not a valid birth certificate). Birth certificate applications are available on line at: Kentucky Birth Certificates.

  • A current Kentucky immunization certificate from your child's physician or the health department showing all age appropriate vaccines.

  • Student's social security card (applications for an original or replacement social security card are available at: Application for a Social Security Card ).
Kentucky law also requires students entering kindergarten to complete the following medical examinations:
  • A physical examination completed by a physician, advanced registered nurse practitioner, physician assistant or the Early Periodic Screening, Diagnosis and Treatment Services (EPSDT).

  • A vision examination completed by an optometrist or ophthalmologist.

  • A dental examination completed by a dentist, dental hygienist, physician, registered nurse, advanced registered nurse practitioner or physician assistant (this exam will be offered free of charge on kindergarten screening dates that will be scheduled at registration).
The exams must be recorded on Kentucky Department of Education forms which are available at medical, optometry and dental offices. Parents are asked to bring these completed exam forms to registration or turn them in soon thereafter. Physical and vision exam forms completed for Head Start or preschool can be used to meet the kindergarten physical and vision exam requirement.

School assignments and parent notifications will be made after registration. Student Enrollment Procedures addressing school assignment is available on the Adair District web site under the Student Procedures section at Adair County School District Board Procedures.
