KSP releases name of victim of fatal collision Update on Wreck north of Lebanon, KY By TFC William Gregory, Public Affairs Officer Kentucky State Police Post 15, Columbia, KY Columbia, KY 24-year-old Melanie A. Mattingly, of Springfield KY, was pronounced dead at the scene as a result of injuries sustained in the two vehicle collision. Mattingly was operating a 2000 Buick 4 Door heading north on KY 0055. Her vehicle was struck head-on when a southbound 2002 Ford Van operated by 31-year-old Marion T. Greathouse, crossed the centerline. Greathouse, from New Haven, KY, was transported to Springview Medical Center, in Lebanon, KY, where he was treated and released.
