Sam D. Lee, 87

Sam D. Lee, 87, a resident of Summit Manor Nursing Home, passed away Wednesday, September 1, 2004, at 3:00 p.m., at the Westlake Regional Hospital in Columbia, KY, after an illness.

The funeral service will be Saturday, September 4, 2004, at 2:00 p.m., at Stotts-Phelps-McQueary Funeral Home, Columbia, KY, with Bro. Raymond Martin, Bro. Terry White, and Rev. Freeman Reece officiating. Interment will be in Haven Hill Cemetery.


He was the son of the late Richard and Eliza Jan Cooksey Lee of Russell County, Kentucky.

Survivors include his wife, Louvern Wilson Lee, Columbia, KY, and the following:
  • Two sons: Delphus, and his wife Becky Lee, Columbia, KY; and Eddie Lee and Elaine Sinclair of Columbia, KY.
  • One daughter: Phyllis, and her husband, Billy Caldwell, Columbia, KY
  • Eight grandchildren: Brian Lee, Craig Lee, Barbie Miller, Lindsey Lee, Chelsey Lee, Bradley Caldwell, Stephen Caldwell, and Jennifer Caldwell
  • One great grandchild: Kaitlyn Miller
Mr. Lee served int he US Army. He was a retired farmer and a member of the Columbia Christian Church.

The family requests visitation after 4:00 p.m. on Friday, September, 3, at Stotts-Phelps-McQueary Funeral Home, Columbia, KY.

Expressions of sympathy may take the form of a donation to the Debt Retirement Fund of the Columbia Christian Church in memory of Sam Lee. He was preceded in death by one great grandchild, Brooklyn Lee, and by the following brothers: Robert, chalie, Oakley, Othie, Ollie, and Willie Lee.
