Roy Cook, 67 Roy Cook, 67, 3548 Allen Schoolhouse Road, Columbia, KY, passed away suddenly on Monday, September 15, 2003, at 12:15 p.m., at the Westlake Regional Hospital Emergency Room in Columbia, KY. The funeral service will be Thursday, September 18, 2003, at 1:00 p.m., at Stotts-Phelps-McQueary Funeral Home in Columbia, with Bro. Richie Curtis officiating. Mr. Cook was retired from Gaddie Shamrock Quarry in Columbia, KY. He was the son of the late George and Flora Harmon Cook of Adair County, KY. He is survived by the following:
