Samuel Otis Lee Phillips passed away Monday October 31, 2011 at Sunrise Hospital in Las Vegas, Nevada. He was 56.

Funeral services are 1pmET/12 noon CT, Friday November 11, 2011 at McKinney-Brown Funeral Home, 752 Campbellsville ST, Liberty, Ky,m with Bro. Burl King officiating. Burial is at Bethelridge Cemetery. Visitation is 6pm-9pmET/5-8pmCT, Thursday, November, 10, 2011, at the funeral home.

He was born in Harlan County, KY on Monday, November 1, 1954. He was the son of the late Samuel Junior and Ruby Osborne Phillips. He passed away Monday, October 31, 2011 in Las Vegas, Clark County, Nevada, at the age of 56 years, 11 months, and 30 days.He was a truck driver, sawmill worker and a self-employed brick mason for 25 years.

Survivors include the following:
  • Two sons: Samuel Lee (Jessica) Phillips of Kings Mountain, KY; and Jonathan (Tiffany) Phillips of Middleburg, KY

  • The mother of his children: Della Phillips Haggard

  • Two brothers: Charles and Steven Phillips of Oak Ridge, Tennessee

  • Two sisters, Sherry Reed and Melissa Phillips both of Oak Ridge, , Tennessee

  • Six grandchildren: Chance, J. C., Dalton, Chase, Wade and Cole Phillips
  • . In lieu of flowers, the family is asking for donations to be given to the funeral home to help offset funeral expenses.

    Funeral arrangements entrusted to McKinney-Brown Funeral Home, 752 Campbellsville Street, Liberty, KY.
