U.S. Dept. of Transportation cites Columbia Gas Transmission

The Pipeline and Hazardous Materials Safety Administration (PHMASA) issued a corrective order to Columbia Gas Transmission on February 14, 2014, the day of the Knifley Pipeline Explosion, ordering Columbia Gulf Transmission Company to take immediate action which included cessation of the operation of the isolated segment until authorized to do so by the PHMSA and to immediately reduce pressure along the entire length of the pipeline.

PMHSA outlined incremental restart plans and outlined return to service protocol.

The PMHSA directive gives the Columbia Gas 90 days to complete a root cause failure analysis. The company is alos ordered to review and assess its public awareness plan. Smith said in a telephone interview that he expects the story to grow in the coming days. "From people I have talked to," he said, "the people of Knifley are just beginning to realize the horror they went through. Their well being should be the focus of our attention now."
