Phyllis S. Travis, 76, of Columbia, KY died Thursday, February 21, 2013 at 11:55pmCT, at the Westlake Regional Hospital Emergency Room, Columbia, KY.

Cremation has been chosen and a memorial service will be held at the convenience of the family.

She was born Tuesday, March 19, 1936, in Camden, Camden County, New Jersey. She was the daughter of the late Ulysses S. and Florence Marshall Travis. She died Thursday, February 21, 2013, in Columbia, Adair County, KY, at the age of 76 years, 11 months and 2 days.

In addition to her parents, she was preceded in death by her sisters; Edythe Wortherley, Romaine Gibson a Winifred Williams. She was a member of Kaighn Avenue Baptist Church in Camden, NJ and a homemaker.

She is survived by her daughter, Karen Travis-Eaddy and her friend, John Pogozelski both of Columbia, KY.
