Driver, passenger, injured when van rear-ended Collision site approximately 11 miles east of Campbellsville By TFC Billy Gregory, Public Information Officer Kentucky State Police Post 15, Columbia, KY LEBANON, KY - Tpr. Casey Wethington is investigating a collision that occurred today, 8 miles west of Lebanon on US 0068. 81-year-old James K Fox of Springfield, KY, was traveling west on US 0068, operating a 2003 Chrysler Van when he rear ended a 2006 Ford.The Ford was operated by 52-year-old Janice Mattingly of Campbellsville, KY. Mattingly was westbound and stopped in traffic, attempting to make a left turn.
