Jacob Tyler Montgomery, 18 months, 4579 Christine Road, Columbia, KY, died Tuesday, September 27, 2005, at 2:01 p.m., at Westlake Hospital Emergency Room from injuries sustained as a result of being struck by a pickup truck on Christine Road.

The funeral service will be Friday, September 30, 2005, at 1:00 p.m., at Stotts-Phelps-McQueary Funeral Home, with Bro. Wayne Keith officiating.


He is survived by his parents, Jason Shane and Jessica Ann Blair Montgomery, and by the following:
  • One brother: Kenneth Montgomery

  • One sister, Dakota Montgomery

  • Paternal grandparents: Kenneth and Marilyn Corbin Montgomery, Columbia, KY

  • Maternal grandmother: Mary Little Tungate, Mannsville, KY

  • Paternal great grandmother: Delpha Corbin, Columbia, KY

  • Maternal great grandmother: Mary Little, Lexington, KY

  • Three uncles: Daniel Kevin Montgomery, Daniel William Blair, and James Anthony Blair.

  • Four aunts: Shannon Duggins and her husband Michael; Hope Freeman and her husband Frank; Ashley Nicole Tungate and Julie Blair

  • Six cousins and a host of other relatives and friends.
Interment will be in Haven Hill Cemetery.

A memorial fund for Jacob Montgomery has been established at the Bank of Columbia.

Visitation will be at Stotts-Phelps-McQueary Funeral Home, 210 Greensburg ST, Columbia, KY, after 4:00 p.m., Thursday, September 29, 2005.
